Registration Opens the public : Saturday, February 1st at 9am!

All 2025-2026 school year forms will be sent electronically, within 4-6 weeks,

to be completed once your child’s application is received and they are placed on a roster.

If you need to update or resubmit your child’s health records, please download and complete the forms below and return them to Page 1 Preschool's main office.

Health and Immunization Record - must be completed by your child’s physician and received in the preschool office by August 1st.
